Moved? New Phone Number? Let H.R. Know.

An employee whose telephone number or address has changed should complete an Employee Information Change form. Employees then submit the completed form to Human Resources.

The form is attached here: Employee Information Change.  Employees can also find the form on the employee tab of Panther Central,  under forms.
